Crosslinks is a not-for-profit organisation that provides disability support to people in the eastern suburbs of Perth and surrounds.
One of our core services is to help people with a disability link to their local community. Our Community Inclusion service promotes independence, inclusion, choice and fun.
As a registered National Disability Insurance Scheme (NDIS) provider, we start with the goals in your NDIS plan and work together to achieve them.
Choose what you’d like to do
Perhaps you’d like to become more confident and familiar with your local area? Whether it’s a visit to the corner coffee shop, a walk in the park, heading to the local recreation centre – or whatever you choose to do – we are here to help. Our qualified, caring Support Workers provide one-on-one, individualised support, to help bring out your best and ensure you have a great day.
Our Community Inclusion service is all about providing person-centred care. We help identify the places, interests and people that are important to you and we work with you to achieve the goals in your NDIS plan and have experiences that you enjoy.
Perhaps your NDIS plan indicates that you’d like to make new friends? If you’re over 18, our RAP Social Club is a great opportunity to meet people and participate in activities, or if you’re 17 and under, check out our Youth Club.
Perhaps there are days when you’re not sure what you feel like doing? Our experienced Support Workers have lots of great ideas plus Crosslinks runs regular, fun activities like drumming, art or moving to music that you can join in – if you wish.
