At Crosslinks your needs are met at every step of the way. We know a cut and paste answer just isn’t enough when it comes to you and your individual requirements and this is why we take the time to implement a rigorous process to help you get the most out of your involvement with us. When we better understand your needs and how you want to spend your time, we are able to build a comprehensive plan that strikes the balance of care, inclusion and fun. Together we’ll go through 3 important stages: 

1. Intake

Here we get to know a lot more about you. This is the start of our relationship so you’ll find us asking lots of questions about your likes, preferences and things you want to achieve. This is our chance to connect with you, understanding what you really want, exploring how we can facilitate your goals in the best way possible. This is also the time when we’re considering a match in a support worker for you. This initial intake interview may sometimes take a couple of meetings, but it’s all about finding the right fit and outcome. After all you’re more than how you present on paper.

2. Service Agreement Paperwork

Once we get a good understanding of who you are, we begin developing your support profile. This ensures your safety during your time with Crosslinks. It also gives your support workers a detailed rundown of your health needs, how you communicate and things that make you, well, you. Like how you might react in a big group or in situations when you’re faced with a personal roadblock. It’s at this stage we present possible support workers and find out any preferences you might have.

Now we develop your Goal Plan, looking at the goals you would like to achieve, creating a strategy and measurable markers around them. Part of this is providing a funding breakdown and any support you might need to navigate the NDIS.

3. Engage our service

Now for the fun bit where we begin our services with you. Through an initial period, we track how you’re responding to your personalised plan and adjust anything that might not be working. Then over time we actively respond to any changing needs that arise, providing clear communication and updated strategies that may benefit you. We will continually check in with you to make sure you’re happy and getting the most out of your experience with us.
