Crosslinks provides long-term Supported Independent Living (SIL) accommodation for NDIS participants. We call our SIL service, “Community Living”. 

Our disability supported accommodation provides shared living spaces and 24×7 supports for people with disabilities. We go to great lengths to ensure participants feel comfortable and have a happy, welcoming, permanent home, with housemates that enjoy each other’s company. 

We also ensure there are appropriate supports in place for every individual. Crosslinks Disability Support Workers are capable, kind, and reliable. Our goal is to provide the right level of care, while also supporting and encouraging people to live as independently as possible.

Are you thinking about moving out of home for the first time? Perhaps your current accommodation is no longer suitable, or you’d like to have housemates to socialise with. 

If you have Supported Independent Living (SIL accommodation) funding in your NDIS plan, we can help. Our person-centred approach links your preferences, needs, personality and NDIS funding to a Supported Independent Living option that’s best for you. We work with your Support Coordinator, family member or guardian to address the accommodation needs in your NDIS plan. 

And if you’re feeling a little scared or overwhelmed about moving homes, that’s OK too! From the first chat to finally moving in to your new home, we will support you every step of the way and make sure you are completely comfortable with the pace of change and the process.

YOUR NEEDS ARE OUR PRIORITY

Crosslinks caters for mild to complex disabilities so whether you need 24×7 support, overnight support, or more occasional care, we can assist. Crosslinks has a team of more than 220 staff and our Disability Support Workers are qualified, caring, kind, and reliable. We can provide the right level of at-home support to suit you.   

A. If you need
constant support

Our experienced Disability Support Workers can be rostered 24 hours a day, 7 days a week to provide full-time support and help with daily living.

B. If you are semi-independent
but would like support at night

Our caring Disability Support Workers often work from dinnertime and stay overnight, to provide assistance and peace-of-mind during the night. 

C. If you're quite independent but
would like some support & companionship

If you’re living alone or need additional support, our friendly Disability Support Workers can regularly visit, to help you achieve the goals in your NDIS plan as part of our Community Participation service.  

About your new home

Our aim is to provide you with a long-term home – a place where you feel secure, cared-for and comfortable. A place where you can entertain family and friends or have some quiet time alone. A place that you can personalise to reflect who you are. A place where you can stay for years, knowing that your particular physical and health requirements are fully catered for.

Our Perth disability accommodation is modern, located in well-established neighbourhoods, and are perfectly set up for people with disability. They are warm and welcoming, with open-plan living areas and outdoor spaces, perfect for socialising with your housemates.

SIL accommodation is shared and housemates are carefully chosen based on criteria like age, gender, interests, disability types and most importantly, overall compatibility with each other.

Making the move

We appreciate that moving into a new home is a big step. That’s why we use a gradual transition process to make the move as smooth as possible. You will be fully supported by our close-knit team consisting of the Community Living Area Manager and your Disability Support Workers. All the team know each participant personally and are always willing to help.

When transitioning to disability supported accommodation, the process is guided by you and your new housemates. The following steps can be taken relatively quickly or relatively slowly, depending on everyone’s comfort level.

SIL accommodation is semi-furnished and are provided with communal-use items like a washing machine, dining table, lounge suite, television, internet, Netflix, kitchen appliances etc. However, you need to provide your own bedroom furniture and linen including:

Looking after your home

Crosslinks advocates for independence and skill-building within people’s capabilities. As it’s your home, you will lead the daily running of the house with your housemates, where possible. This may include cooking meals, checking the mailbox, grocery shopping, putting the washing on, taking the rubbish bins out etc.

Living in your own home provides amazing opportunities to learn and grow and your Disability Support Workers are always there to support and assist you.

 

Looking after yourself

You will have a personal support profile that reflects the level of independence that you’re comfortable with. You will have the opportunity to learn to manage all areas of your own personal care such as showering, bathing, teeth cleaning, hair washing, toileting, washing hands, maintenance of nails etc. Of course, full support is provided if required.

 

Meals

You will be invited to become involved in meal planning with your housemates and Support Workers. Weekly menus will be developed based on what everyone in the house enjoys eating, people’s specific dietary requirements and nutrition. You will also be encouraged to assist the Support Workers in the preparation of meals as it’s a great learning opportunity – and it’s fun!

Perhaps once a week all the housemates would like to go out for dinner? Perhaps you’d like to use the BBQ at each home to have a regular BBQ? These are the types of independent-living decisions that you and your housemates can make, with help from your Support Workers.

Fees and Contributions

Fees for the cost of living in the group home and the delivery of support services vary, depending on your needs and the number of people living in the home. The cost of living in disability supported accommodation will be covered by your disability support pension or wages. The cost of support services will be based on your NDIS funding.
